A&S Compassion Care provides quality person-centered care, where the individual’s needs are prioritized at the forefront. Our Home Health Services are available for Adults 18 years of age and older, who may need such services as companion care, personal care, and transitional care.
Much of the work we do is focused on reducing unnecessary health service utilization, such as emergency room visits, observation, and inpatient admissions. We work with hospitals, medical practices, and other managed care organizations to provide care coordination that is customized for the individual. This may include services such as Skilled Nursing.
Our qualified and carefully screened Caregivers, CNA’s, and LPN’s can provide care around the clock or care for just a few hours a week. All of our home health staff are consistently supervised by our Registered Nurses and Director of Nursing to ensure the highest standards of quality care. We carefully recommend and match our individuals with caregivers based on mutual compatibility.
Private Pay plans are available that work best for the individual and their budget.
