Transition Services provides funding for certain expenses if an individual moves from an institutional setting to a group home, or if an individual moves from a provider operated home to their own home in the community.
- Includes expenses such as security deposits, fees or other deposits needed to set up utilities, and moving expenses.
- Individuals must meet certain requirements, which should first be discussed with the individual’s Coordinator of Community Services (CCS), and there must be a documented need in the individual’s Person-Centered Plan (PCP).
